(Well, don’t “get it” get it with the self-mutilation part, but I can see the connection.) Ditto for a person wrapped in barbed wire as a symbol of repression or oppression. Either image would work well in a political cartoon, for example. You could show someone a drawing of a person with lips sewed shut (e.g., http://efplfp.stealingisgood.com/bandaid.html), ask someone what it was a protest against, and get a reasonably good answer.

But nailing your balls to the road? I don’t see the connection. Show someone a picture of him and ask what he’s protesting, and you’ll get a bunch of shrugged shoulders and quizzical looks.

When the protest isn’t designed to actually be effective, you have to wonder what the person is really looking to accomplish.
